It is caused by a <strong>malfunction of the lymphatic system<\/strong>, which impairs the drainage of interstitial liquids, leading to an <strong>increase in volume <\/strong>of the affected area, associated with <strong>skin hardening <\/strong>and, in some cases, <strong>pain or reduced mobility.<\/strong><\/p><p class=\"translation-block\">The lymphatic system, in fact, is responsible for <strong>three main functions<\/strong> in our organism:<\/p><ol><li class=\"translation-block\"><strong>Drainage of interstitial fluids<\/strong>: the network of vessels that form this system collects <strong>lymph<\/strong>, a tissue-waste fluid containing white blood cells, and carries it to lymph nodes to eliminate toxins, metabolic waste and excess fluids, maintaining hydric balance and preventing stagnation and oedemas.<\/li><li class=\"translation-block\"><strong>Transfer of fats and nutrients<\/strong>: it absorbs lipids and fat-soluble vitamins from the small intestine and transports them into bloodstream.<\/li><li class=\"translation-block\"><strong>Immunologic response<\/strong>: lymph nodes filter lymph, trapping pathogens and activating immune defences via lymphocytes.<\/li><\/ol><p class=\"translation-block\">Under normal conditions, lymph flows through the lymphatic vessels thanks to muscle contraction and one-way valves, to ensure a <strong>steady balance between the lymphatic and the circulatory system<\/strong>.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b76a2e1 elementor-widget elementor-widget-heading\" data-id=\"b76a2e1\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Main causes and risk factors<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-28cddb4 elementor-widget elementor-widget-text-editor\" data-id=\"28cddb4\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p class=\"translation-block\">There are two main typologies of lymphoedema. <strong>Primary lymphoedema<\/strong> is a <strong>congenital or hereditary condition <\/strong>caused by malformations of the lymphatic system. Depending on the stage of life in which it may manifest itself, we make a distinction among <strong>congenital lymphoedema<\/strong> (<em>present at birth<\/em>), <strong>early lymphoedema <\/strong>(<em>arising during childhood or adolescence<\/em>) and <strong>late lymphoedema<\/strong> (<em>appearing after the age of 35 without a clear cause<\/em>). <strong>Secondary lymphoedema<\/strong>, on the other hand, is caused by <strong>damages or obstructions of the lymphatic system<\/strong> as a result of surgery, radiotherapy, traumas, infections, and chronic diseases, such as venous insufficiency, which may impair lymphatic drainage.<\/p><p class=\"translation-block\">There are also some factors that  <strong>may increase the likelihood of developing a lymphoedema <\/strong>, such as surgery with removal of lymph nodes, oncological therapies (radiotherapy, chemotherapy), chronic infections of the skin or subcutaneous tissue, repeated traumas, or medical conditions that impair lymphatic circulation.<\/p><p class=\"translation-block\">Lymphoedema is a <strong>progressive condition<\/strong>, but early diagnosis and appropriate treatments can limit its effects and improve the patient's quality of life.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1c3e032 elementor-widget elementor-widget-heading\" data-id=\"1c3e032\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Stages, symptoms, and signs<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-6c6ac5f elementor-widget elementor-widget-text-editor\" data-id=\"6c6ac5f\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\tLymphoedema is a progressive disease that evolves in several stages. <br>According to the classification of the <em> International Society of Lymphology<\/em> (ISL), there are <strong>four stages<\/strong>:\n<br>\n<br>\n<ul><strong>Stage 0 (subclinical lymphoedema) <\/strong>\n<br><li style=\"list-style-type: none;\">The lymphatic system is damaged, but there are no obvious signs of swelling or changes of the skin tissue yet. <br>Feeling of heaviness or tension in the limb may occur.<\/ul>\n<ul><strong>Stage 1 (early lymphoedema) <\/strong>\t<li style=\"list-style-type: none;\">\nThe oedema is visible but tends to disappear spontaneously with rest and elevation of limbs. The skin is still soft and pressure with a finger leaves a temporary dimple (<em>Fovea Sign<\/em>). Lymphatic fluid accumulates, but no permanent tissue changes are observed.<\/ul>\n<ul><strong>Stage 2 (mild lymphoedema) <\/strong>\n\t<li style=\"list-style-type: none;\">\nThe volume of oedema increases; it gets permanent and does not disappear with rest. The skin changes its consistency, becoming thicker and harder due to tissue fibrosis. The Fovea Sign may be less visible or absent.<\/ul>\n<ul><strong>Stage 3 (severe lymphoedema)<\/strong><br>\nThe limb is significantly enlarged and misshapen, even to elephantiasis. The skin changes; it is thickened, with skin alterations  and folds. Skin ulcers and recurrent infections are possible. The mobility of the limb may be impaired.<\/ul>\n<br>\nSymptoms may vary depending on the stage, but the most common are:\n<br>\n<br>\n<ul>\n \t<li class=\"translation-block\"><strong>Progressive swelling<\/strong> usually of a limb, but may also affect the torso or other areas<\/li>\n \t<li class=\"translation-block\"><strong>Feeling of heaviness or tension <\/strong>in the affected area<\/li>\n \t<li class=\"translation-block\"><strong>Alterations of the skin, <\/strong>which may become harder, drier or less elastic<\/li>\n \t<li class=\"translation-block\"><strong>Pain or discomfort, <\/strong>especially in advanced stages<\/li>\n \t<li class=\"translation-block\"><strong>Reduced mobility <\/strong>of the affected limb<\/li>\n<\/ul>\nRecognizing the early signs is crucial for a timely intervention. Early diagnosis by a medical specialist and intervention with appropriate and controlled treatments can slow down the progression of the disease and improve the quality of life of sufferers.\n<br>\n<br>\n<ul>\n \t<li class=\"translation-block\">Discontinuous<strong> swelling sensation, <\/strong>especially at the end of the day<\/li>\n \t<li class=\"translation-block\"><strong>Difficulty in putting on rings, shoes, or clothes <\/strong>in the affected area<\/li>\n \t<li class=\"translation-block\"><strong>Fovea Sign<\/strong>,the pressure with a finger leaves a temporary mark on the skin<\/li>\n \t<li class=\"translation-block\"><strong>Increased sensitivity or stiffness <\/strong>of the limb compared to the opposite side<\/li>\n \t<li class=\"translation-block\"><strong>Alterations of the skin, <\/strong>which may appear shinier or more stretched.<\/li>\n<\/ul>\n<em>If you have any suspicion that you might have a lymphoedema, see a specialist for an accurate diagnosis.<\/em>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cd14efd elementor-widget elementor-widget-heading\" data-id=\"cd14efd\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Diagnosis<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0641897 elementor-widget elementor-widget-text-editor\" data-id=\"0641897\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p class=\"translation-block\">An <strong>early diagnosis <\/strong>of lymphoedema is crucial to prevent complications and start an appropriate treatment in a timely manner. The diagnostic process is based on a thorough <strong>clinical evaluation supported<\/strong>, if necessary, <strong>by instrument tests <\/strong>to confirm the diagnosis and determine the extent of lymphatic damage.<\/p><p class=\"translation-block\">The first diagnostic phase is <strong>anamnesis<\/strong> and <strong>physical examination<\/strong>, during which the doctor gathers information on the patient's medical history (family history, surgeries, traumas, infections) and analyses the reported symptoms. <strong>Inspection<\/strong> and <strong>palpation of the affected area<\/strong> further allow for the detection of characteristic symptoms of lymphoedema, including the possible presence of hardened skin, the <strong>Fovea Sign <\/strong>(persistence of the imprint after skin pressure) and differences in circumference between limbs.<\/p><p class=\"translation-block\">If the clinical assessment is not conclusive or a more in-depth analysis is required, <strong>instrumental investigations<\/strong> are performed, including:<\/p><ul><li class=\"translation-block\"><strong>Lymphoscintigraphy<\/strong>, a reference exam for the study of lymphatic flow, useful for identifying any obstructions or alterations in the lymphatic circulation.<\/li><li class=\"translation-block\"><strong>Tissue ultrasound scan<\/strong>, used to exclude other possible causes of oedema, such as venous insufficiency or lipoedema.<\/li><li class=\"translation-block\"><strong>MRI or CT scans<\/strong>, used in the most difficult cases to obtain a detailed view of the lymphatic structure and surrounding tissues.<\/li><li class=\"translation-block\"><strong>Bio-electrical impedance analysis (BIA)<\/strong>, body composition analysis to assess fluid distribution and the degree of water retention.<\/li><\/ul><p>Thanks to an accurate and timely diagnosis, early intervention is possible, thus improving the management of the disease and the patient's quality of life.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b0e45af elementor-widget elementor-widget-heading\" data-id=\"b0e45af\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Treatment<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-879ce81 elementor-widget elementor-widget-text-editor\" data-id=\"879ce81\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>There is no conclusive therapy for lymphoedema, but its treatment aims to reduce swelling, improve the function of the affected limb and prevent complications.<\/p><p class=\"translation-block\">Currently, the most effective treatment is <strong>Complex Decongestive Therapy (CDT)<\/strong>, a multimodal approach that combines several techniques to reduce oedema and maintain results over time.<\/p><p class=\"translation-block\"><strong>CDT<\/strong> consists of <strong>two main phases<\/strong>: the intensive <strong>phase<\/strong> called <strong>decongestive<\/strong>, where the volume of oedema is reduced through daily treatments, and the <strong>maintenance phase<\/strong>, in which achieved results are stabilised to prevent a progression of the pathology. This latter phase is crucial and should be pursued consistently throughout life due to the chronic nature of lymphoedema.<\/p><p class=\"translation-block\">During the decongestive phase, the treatment is based on two essential elements: <strong>Manual Lymphatic Drainage (MLD) <\/strong>and compression by <strong>compression bandaging<\/strong>.<\/p><p class=\"translation-block\">MLD is a specific massage technique that <strong>stimulates the lymphatic system, facilitating the reabsorption of lymph into lymphatic vessels and its outflow towards uncompromised lymph node positions<\/strong>, thus helping to reduce swelling and improve patient comfort.<\/p><p class=\"translation-block\">After drainage, a compression dressing with <strong>short-stretch bandages<\/strong> is applied, which is indispensable for preserving achieved benefits and promoting fluid reabsorption. This type of bandage exerts targeted pressure, supporting lymphatic return and limiting fluid stagnation. In the intensive phase of treatment, bandages must be kept in place 24 hours a day to ensure maximum therapeutic effectiveness.<\/p><p class=\"translation-block\">During the maintenance phase, compression bandaging is replaced by daily wearing of flat-knit graduated compression stockings, designed to ensure the effective containment of oedema, and stabilise limb volume. Flat-knit stockings are characterised by their rigid and less elastic structure compared to circular-knit stockings; they allow to exert a <strong>constant and uniform therapeutic pressure <\/strong>to counteract the accumulation of fluids. Model and compression class should always be determined by a specialist, depending on the stage of the pathology. Thanks to their high degree of resistance and capacity to maintain therapeutic pressure over time, they are an indispensable solution for a long-term control of the pathology.<\/p><p>A timely management of lymphedema is essential to reduce the risk of exacerbations. It is important to avoid high-impact exercises or excessive strain.\n<br>\n<br>\n<strong>Skin care and hygiene<\/strong><br>\nThe skin of people with lymphedema is more fragile and prone to infections, such as erysipelas. It is essential to adopt a correct hygiene and protection routine. Always keep your skin hydrated, avoid traumas, insect bites and wounds, which could represent a gateway for infections.\n<br>\n<br><strong>Patient education and lifestyle<\/strong><br>\nTo keep lymphedema under control it is essential to adopt correct daily habits.
